HTML_QuickForm_Combobox in an element of PEAR HTML_QuickForm package.
To install you have two choice:

1. download combobox.php file and put it into HTML/QuickForm subdirectory under pear directory

2. download the entire package archive from my site www.ambrosanio.com and install it using "pear install" command
