quick install of mod_evaluation:

Install phpsurveyor as usual.
copy mod_evaluation directory below the phpsurveyor directory.
change the evaluation.config.php to your needs.
run the current_db_dump.sql on your phpsurveyor database (clean the phpsurveyor db first)
