Upload on server, navigate to that DIR, 

You will be asked for registration params username and password -> register
-> login with that params
-> enter database params
-> go wild!!

NOTE: DIR [Auth_simple] and [db2mysql2pro] must be writeble at least first run or if you whant to change username and pasword and database
